Infant Memorial Engraving Request Form

The St. Michael the Archangel Parish Cemetery has a special monument in memory of all babies born into Heaven. Those who wish to have their child's name engraved on the monument are invited to submit this form.

Engraving requests will be submitted to the engraver on the second Friday of each month. It may take up to a month from your submission for the engraver to add a name to the monument. After submitting a form, please send a check for $20o made payable to "St. Michael Cemetery" to the Parish Office in an envelope marked "St. Michael Cemetery - Parish Office" and we will process your request.
